By completing and signing the application below, you agree to the program’s terms and conditions on the back of this form. Please address any questions or concerns with a PAWS representative before signing. Concho Valley PAWS agrees to spay and vaccinate the mother dog at no cost to you as long as the puppies are safely cared for until placement.
